Report Must Be Submitted Not Later Than 24 Hours From The Completion Of The Game(s) In Question. If using this form to report an ejection or disciplinary action, the official that actually ejected the player(s) and or coach (s) should be the one completing this report. Make sure you list all facts, player jersey numbers. time of ejection and what NFHS rule(s) were violated. Example. " At 5:22 in the first quarter, player #44 from The Vikings was ejected for striking a Cardinals player in violation of NFHS rules. Player was escorted to his team bench, head coach was informed and the game continued without further incident."
